Document Attestations from Ministry of Health (MOH)

MOH attestation provides legal validity of medical reports or medical letters. If these document require MoFA attestation or Apostille, MOH attestation is mandatory before proceeding it.

Requirements
Should Know

Provide the original medical document which follows the criteria and copy of Saudi national ID/ Iqama in GA center.

Original medical document.

Document is printed in the official format of the hospital with its logo
Document must be typed, not handwritten
Name, stamp and signature of the doctor whose signature is registered in MoH system
Official hospital stamp with its logo is stamped

If the use is for Apostille attestation, please inform GA agency when providing the documents since it requires different procedure.
